I agree to participate in the MobiCare Plus insurance program under the MC720 package I registered with MobiFone. I confirm that I have carefully read and accepted all the terms and conditions of the MobiCare Plus Insurance Program provided by PVI Insurance, and I also confirm that I do not fall under any of the following excluded categories:
- Under 16 years old and/or over 60 years old;
- Working in physically demanding or dangerous jobs such as mining, tunnel digging, underwater work, diving, fishing at sea, criminal investigation police, special task forces, stunt acting in dangerous scenes, military service (except for office roles), or jobs involving explosives like bomb disposal police or demolition workers;
- Suffering from mental illness, leprosy, cancer; having a disability or permanent injury of 50% or more; currently undergoing treatment for any illness or injury; or in the process of preparing for medical tests or diagnosis for symptoms or an unspecified illness;
- Currently under medical quarantine and/or receiving treatment for Covid-19 as mandated by health authorities or local government.
I commit that:
- All the information I have provided above is completely true and accurate;
- I understand that any false or inaccurate declaration may result in the insurance certificate/contract being invalidated and/or PVI Insurance having the right to deny part or all of any claims in the event of an insured incident;
- I agree to inform PVI Insurance of any changes to my personal information (if any);
- I give my consent for doctors, medical consultants, or healthcare professionals at hospitals or medical facilities where I have received treatment or health consultations to share any health-related information with PVI Insurance if needed for claim verification and processing purposes.

Introduction to MobiCare Plus
The MobiCare Plus Insurance Program is a combined accident, illness, and health insurance plan bundled with the MC720 package issued by MobiFone. It offers customers a great opportunity to enjoy attractive data benefits along with health protection coverage within the territory of Vietnam.
| Article 1 | Eligible Participants for Insurance: All Vietnamese citizens and foreign nationals legally residing in Vietnam between the ages of 16 and 60 are eligible to participate, excluding the following individuals: Those working in physically demanding or high-risk jobs such as mining, tunnel digging, underwater work, diving, offshore fishing, criminal investigation police, special task forces, stunt performers in dangerous scenes, military personnel (except those in office roles), or individuals involved in bomb-related work like bomb disposal police or demolition workers; Those suffering from mental illness, leprosy, cancer; individuals with permanent disabilities or injuries of 50% or more; those currently undergoing treatment for any illness or injury; or those preparing for medical examinations or diagnoses for unclear symptoms or undiagnosed conditions; Those currently being treated for Covid-19 or under medical quarantine at centralized facilities as mandated by health authorities or local government. In the event that an ineligible person participates in the insurance program, the insurance contract/certificate will be rendered invalid, and the insurance premium will not be refunded to the insured. |
| Article 2: | Definitions: Pre-existing Conditions: These are illnesses or medical conditions that existed before the start date of the insurance coverage and meet one of the following criteria for the Insured Person: The condition required treatment within the 3 years prior to the insurance start date; or It had medical symptoms or origins that existed before the insurance contract took effect—regardless of whether the Insured had previously received treatment or medical consultation. Special Diseases: These include cancer and all types of tumors, high blood pressure, cardiovascular diseases, stomach ulcers, chronic polyarthritis, intestinal ulcers, hepatitis (any type), endometriosis, hemorrhoids, kidney or gallstones, cataracts, sinusitis, diabetes, asthma, Parkinson’s disease, nephritis, and conditions related to the blood formation and regeneration system such as dialysis or blood transfusions. Congenital Diseases or Malformations: These refer to abnormal development in the shape, structure, or positioning of an organ or body part that occurs during fetal development in the womb. |
| Article 3: | Insurance Coverage Scope: Death due to illness or disease (including death from vaccination complications and deaths due to epidemics), except for cases excluded under Article 4 below; Death or bodily injury due to an accident, except for cases excluded under Article 4 below; Illness or disease (including treatment for vaccination complications), hospitalization, or surgery due to illness, except for cases excluded under Article 4 below. Insurance Validity: For death due to illness or disease: Insurance is effective after the waiting period from the start date of the insurance coverage stated in the Insurance Contract/Certificate: 30 days for common illnesses; 12 months for special diseases and pre-existing conditions; For death or bodily injury due to an accident: Insurance is effective from the start date of the insurance coverage stated in the Insurance Contract/Certificate. For hospitalization or surgery: Insurance is effective after the waiting period from the start date of the insurance coverage stated in the Insurance Contract/Certificate: No waiting period for common illnesses; For treatment of complications from pregnancy, miscarriage, or abortion (based on doctor’s recommendation) within 90 days from the start of the insurance coverage, or for childbirth within 270 days from the start of the coverage, the payout will be calculated based on the ratio of the days from the start of coverage to the date of the insured event compared to 90 or 270 days; 30 days for special diseases and pre-existing conditions. Benefits for the Insured: In the event of death due to causes covered by the insurance, PVI Insurance will pay the full sum insured as specified in the Insurance Contract/Certificate; In the event of bodily injury due to an accident covered by the insurance, PVI Insurance will pay the compensation according to the “Accident Injury Compensation Table” attached to the MobiCare Plus Insurance Program, based on Decision No. 05/TC-BH dated January 2, 1993, by the Ministry of Finance; For hospitalization covered by the insurance, PVI Insurance will provide daily allowances as follows: For the first 10 days: VND 100,000 per day; For the next 20 days: VND 60,000 per day; For the next 30 days: VND 40,000 per day. The daily allowance for hospitalization will be paid starting from the second day of hospitalization for each hospitalization period. The total number of days for which the allowance is paid per year is continuous and not limited by the number of hospitalizations, with a maximum of 60 days per insurance year. For hospitalization in traditional medicine hospitals or departments, PVI Insurance will pay VND 40,000 per day of treatment, with a maximum of 150 days per insurance year. For surgery covered by the insurance, PVI Insurance will provide compensation according to the “Surgery Compensation Table” attached to the MobiCare Plus Insurance Program. |
| Article 4: | Exclusion Clauses: General Exclusions: Intentional actions by the Insured Person or their legal heirs; The Insured Person, aged 16 or older, committing criminal offenses, violating local laws or regulations, or breaking traffic safety rules while operating a vehicle; The Insured Person using alcohol, drugs, or similar stimulants; Earthquakes, volcanic eruptions, radiation contamination; War, civil war, strikes. Exclusions Specific to Hospitalization or Surgery: The Insured Person being hospitalized for treatment of nervous exhaustion, general weakness, spinal degeneration, spinal disc herniation, or treatment in convalescent homes or nursing homes; Hospitalization for health checks or medical evaluations unrelated to illness treatment; Treatment or surgery for congenital diseases, injuries, or conditions that existed before the insurance coverage started; Treatment or surgery requested by the Insured Person not related to regular medical treatment or surgeries as defined by healthcare regulations; Cosmetic surgery, orthopedics, rehabilitation, the creation of prosthetics (e.g., artificial limbs, eyes, or dentures); Family planning (using medical methods for birth control), infertility treatment, impotence treatment, disorders or dysfunctions of the reproductive system, artificial insemination, hormone replacement therapy, gender reassignment surgery, or any consequences or complications arising from these treatments; The Insured Person being diagnosed with syphilis, gonorrhea, HIV virus infection, malaria, tuberculosis, or occupational diseases. |
| Article 5: | Insurance Amount: The insurance amount is agreed upon and determined based on the Insurance Request Form for the Insured Person and the approval from PVI Insurance; The insurance amount in the event of death is the maximum amount PVI Insurance can pay to the Insured Person in a single insurance event. The insurance amount for hospitalization or surgery is the maximum amount PVI Insurance can pay to the Insured Person in a single insurance year for each hospitalization or surgery case. The insurance amount is specified in the Insurance Contract/Certificate. |
| Article 6: | Responsibilities of the Insured: Provide truthful and complete information for all items specified on the Insurance Request Form of PVI Insurance and pay the insurance premium as per the terms of the Insurance Contract/Certificate; In the event of an insured risk occurring, the Insured or their relatives must take necessary and feasible measures to assist the victim; Be truthful in declaring and providing accurate documents regarding the insured event. |
| Article 7: | Procedure for Insurance Enrollment: The MobiCare Plus product is integrated into the MC720 package of MobiFone. After successfully subscribing to the MC720 package and reading the confirmation of insurance participation via the link provided in the SMS notification from MobiFone, customers need to: Text “DK MC720” or “MC720” to 789 to subscribe to the MC720 package of MobiFone; Text “Y” to 789 to confirm agreement to join the MobiCare Plus program. Customers will receive an electronic Insurance Certificate through the link attached in the SMS sent by MobiFone’s customer service confirming the successful registration of insurance. |
| Article 8: | Procedure for Changing the Insured Person’s Information: The MobiCare Plus product is issued based on the information of the registered account holder. If the subscriber’s information is found to be incorrect, PVI is responsible for assisting the customer in correcting or adjusting the information (free of charge) on the insurance application according to the terms of the insurance program. The information that can be corrected or adjusted includes: full name, date of birth, gender, email, address, Citizen ID/ID card number, but no more than 3 pieces of information can be corrected or adjusted per insurance application for the same customer. In case the customer detects incorrect information before an insurance event occurs: Contact PVI Insurance’s Claims Support Staff at: 0932.692.381 for assistance in adjusting the information. In case the customer detects incorrect information after an insurance event has occurred: Attach a copy of the ID card/Citizen ID to the claim application and send it to PVI Insurance to request correction or adjustment of the information. |
| Article 9: | Claims Procedure and Required Documents: When requesting insurance payment, the Insured or their representative or legal heirs must submit the following documents to PVI Insurance within 30 days from the date the Insured is cured, discharged from the hospital, or passes away (but not more than 1 year from the date the insured risk occurs): Insurance claim request form (according to PVI Insurance’s template) along with the most recent copy of the customer’s ID card/Citizen ID; Accident report certified by the local authority or police where the Insured had the accident, and a copy of vehicle documents including: driver’s license, vehicle registration, vehicle inspection certificate (for motorcycle or car accidents); Original or certified copy from the hospital of medical treatment documents: discharge letter, treatment receipts (for inpatient treatment), surgery receipt (for surgery), and any related invoices or documents (if available); Death certificate and legal heir certificate (in case of the Insured’s death). | Article 10: | Insurance Payment Settlement: The insurance payment will be paid by PVI Insurance to the Insured, their legal heirs, or a legally authorized representative. In case the Insured, authorized representative, or legal heirs are found to be dishonest in complying with the terms and conditions of this Insurance Program, PVI Insurance reserves the right to deny part or all of the insurance payment depending on the severity of the violation. PVI Insurance is responsible for reviewing, resolving, and paying the insurance claim within 15 days from the date of receiving all valid documents. If additional verification is required, the payment will be made no later than 30 days from the date of receiving the complete and valid documents. In case the Insured holds multiple Insurance Contracts/Certificates under this insurance program, when an insured event occurs, PVI Insurance will pay the insurance claim according to the contract with the most recent effective insurance period. |
| Article 11: | Complaint Period and Dispute Resolution: The complaint period for PVI Insurance regarding the resolution and payment of the insurance claim is 06 months from the date of receiving the claim settlement notification from PVI Insurance. After this period, any complaints will be deemed invalid. Any disputes related to the insurance contract will be resolved through negotiation. If the parties cannot resolve the dispute through negotiation, either party has the right to submit the dispute to the competent court in Vietnam for resolution. |
